How to Raise a Concern

If you have a concern about the Fitness to Practise of someone on the Accredited Register, you should in the first instance complete an AHCS Complaint & Concern Form, and email it to or return it by post to:

Academy for Healthcare Science
6 The Terrace, Rugby Road,
Lutterworth, Leicestershire LE17 4BW

Head of Operations will acknowledge receipt of the completed Form within 3 working days, and refer it on to the Regulation team.

Please ask a member of the team if you require assistance filling out the form, you can also ask a personal representative to do it for you if you prefer.
